    Municipal Long-term Care Service Application Counter Print

    1. Residents living in this city, who are over the age of 65, hold a certificate of disability, aboriginals from the age of 55 to 64, and individuals over the age of 50 who have dementia (relevant information such as disability card, diagnosis certificate, physician opinion letter, etc. need to be provided) that require long-term care service, please submit an application to the substations of the Municipal Long-Term Care Service Management Center. The care manager will visit and conduct an overall assessment. The long-term care service will be provided once the care management system determines that the long-term care requirement level is above level 2.
       
    1. The substations of the Municipal Long-Term Care Service Management Center have been stationed in the health centers of various districts since 2017. For those who require long-term care service, please submit an application through the health centers of each district or call the service line 1966.
